Request Removal from Child Abuse Registry
A substantiated perpetrator may request removal from the Department of Children’s Services Child Abuse Registry (CAR). This removal process is not an opportunity to challenge or dispute the facts of the original Child Protective Services case. It is an opportunity for the applicant to provide a justification detailing the facts and circumstances that have occurred since the date of the substantiation demonstrating rehabilitative actions that warrant removal from the CAR as decided by the CAR Review Committee.
When submitting for the review the applicant must:
- Complete the application and submit it directly to the address listed at the end of the application.
- Provide a justification detailing the facts and circumstances that have occurred since the date of the CPS case closure demonstrating rehabilitative actions or other circumstances warranting removal from the CAR.
- Provide an explanation of any arrests and/or convictions of felonies and/or misdemeanors that have occurred since the date of the CPS case closure.
- Provide an explanation of disciplinary actions taken or pending regarding professional or occupational licenses that have occurred since the date of the CPS case closure.
The applicant is encouraged to provide any additional information he or she feels is relevant for the Committee to review and consider.
