About PDF Files
Adobe Acrobat's portable
document format (PDF) files are widely used on the Web because they preserve
the original appearance of the printed document. PDF is a universal file format
that preserves all the fonts, formatting, graphics, and color of any source
document, regardless of the application and platform used to create it. Adobe
PDF files are compact and can be shared, viewed, navigated, and printed exactly
as intended by anyone with Acrobat Reader software. The software is free of
charge and is available by download from adobe.com on the web.

Online Forms
Viewing: For easier
viewing, click on the Acrobat Reader's Zoom button and then click anywhere on
the page.
Fill-In Forms: To complete
forms online, use the "Tab" key to move through the fields or click
on a field using the mouse. When tabbing between checkbox fields, you must
press the space bar or click the box with the mouse to mark the field. Once the
form is complete, print using the printer icon from the Acrobat toolbar.
Submit Form: Mail the
completed form with any necessary documentation to the appropriate address.
