RELEASE OF INFORMATION TO THE APPLICANT/RECIPIENT OR HIS/HER REPRESENTATIVE |

The client or representative can request
information be printed from his/her eligibility and case management system
record. A charge of 10 cents per page
may be made except when being secured for a fair hearing. Upon proof that an attorney has been secured by the client, the
attorney or an authorized non-lawyer employee will have access to examine,
request print, or copy information in the file. The file should be documented by the
caseworker when this occurs. If a client or representative is viewing the record on-line, the
caseworker must be present. Information considered “Confidential” by the provider and potentially
damaging to the client should be provided after DHS informs the individual or
his/her attorney that it is inadvisable and the individual or attorney
continues to request access to the information. |
