  1. Can I remain anonymous if I call to report someone for tax fraud?
    Yes. You do not have to give your name, phone number or any identifying information. It may be helpful for the agent to be able to contact the person reporting the fraud if they need more details; however, this information is not required.
  2. How do I report someone that I think is committing tax fraud?

  3. If I choose to write the information down, is there a form for me to fill out so that I can get you the details you need?
    Yes. Click here to complete the form online. Click here to obtain a PDF version of the form. Print, complete and mail the form to the Department of Revenue. The form will give you a good indication of the information you will be asked about if you were to call the office. You can mail the form to the address above, or fax it to us at (615) 532-1535.
  4. What information do I need in order to report someone?
    This varies greatly from case to case. The more details you can provide the easier it is to evaluate the facts and circumstances surrounding the issue(s). There are several things that are important to all cases:
    • Who was involved?
    • What did they do?
    • When did they do it?
    • Where did it happen?
    • How did it happen?
    • How do you know it happened?
  5. Is there a reward for reporting someone?
    The Department of Revenue currently does not have a reward program for reporting tax fraud. However, please consider the fact that the person committing the crime is not only stealing from the state but in effect from you, too.
  6. If I provide you my name and number, can I receive periodic updates on the investigation?
    No, the rules of confidentiality prevent the department from releasing this information.
  7. Where do most tips of alleged tax fraud come from?
    Spouses, employees, consumers, law enforcement agencies, and other government entities.
