Forms and Instructions for New Filing Procedures

1.	Download and fill out the appropriate form then print 4 copies of rules or 3 copies of hearing notices. Attach an additional redlined copy of rule amendments per Public Chapter 741 (2008). Copies of the entire rule chapter are no longer necessary. 2.	Get the appropriate signatures on all copies, then submit to the Attorney General’s Office for review.
3.	Once a signed copy is returned from the Attorney General's office, submit all copies and a digital file on CD to: Department of State, Division of Publications, 312 Rosa L. Parks Avenue, 8th Floor William Snodgrass Tennessee Tower
4.	A signed copy will be returned to your agency.
