Admissions to Faith Team Ministries Suspended

NASHVILLE – Tennessee Department of Health Commissioner Susan R. Cooper, MSN, RN, has suspended new admissions of residents to Faith Team Ministries, a licensed residential home for the aged, effective December 11, 2008. A special monitor has been appointed to review the facility’s operations.
 
 Faith Team Ministries, an eight-bed facility located at 667 Bethel Avenue in Memphis, was ordered not to admit any new patients based on conditions found during a complaint investigation conducted December 3. During the investigation, surveyors found violations of the following standards: administration; admission, discharge and transfers; personal services; infectious and hazardous waste; records and reports; resident rights; policies and procedures for health care decision-making; and disaster preparedness. 
 
The State Health Commissioner may suspend admissions to a home for the aged when conditions are determined to be, or are likely to be, detrimental to the health, safety or welfare of the residents. The order to suspend admissions remains effective until conditions have been and continue to remain corrected. A copy of the order must be posted at the public entrance of the facility, where it can be plainly seen. The facility has the right to a hearing regarding the suspension of admissions before the Board for Licensing Health Care Facilities or an administrative judge.
