Lakeshore Heartland Admissions Suspension Lifted

Tennessee Department of Health announces that the suspension of admissions at Lakeshore Heartland nursing home has been lifted.

The facility is a 66-bed licensed nursing home located at 3025 Fernbrook Lane in Nashville. The Department confirmed the facility has returned to substantial compliance for state licensing purposes, and the deficient practices and conditions have been corrected as stated in the facility’s plan of correction, which was approved by federal and state authorities. On June 27, the nursing home was ordered not to admit any new residents based on conditions found during a complaint investigation and survey conducted June 15 through June 19. During the investigation, surveyors found violations of administration, nursing services, medication administration, physician notification, and quality of care standards.
