Tennessee Board of Nursing Suspends Licenses of Coffee County Nurse

NASHVILLE – The Tennessee Board of Nursing has suspended the registered nurse license and advanced practice nurse certificate of Stacy Rayfield, RN, APN. The suspension of the nursing license and certificate became effective June 3, 2010. As a result of the summary suspensions, Rayfield is immediately prohibited from working as a nurse in Tennessee.

Rayfield’s license and certificate were suspended for the following:

• engaging in a pattern of substandard care, inappropriate prescribing, unprofessional conduct and deceptive practices

Rayfield has the opportunity to participate in an informal conference before the Board of Nursing on June 9, 2010, at 3:30 p.m., CT. During the conference, the board will determine whether the public health, safety or welfare imperatively required emergency action. The conference will be canceled if Rayfield declines participation.

Rayfield was originally licensed as registered nurse in Tennessee in 1996. She received a certificate of fitness as an advanced practice nurse in 2004.
