Report Tax Fraud
Violations to Tennessee tax laws can be reported online, in writing or by calling (800) FRAUDTX or (800) 372-8389.
Please follow the steps below if you report tax fraud to the Department in writing:
1. Download the PDF version of the tax fraud form.
2. Please include the following information. Although you may not have all the information asked, the more details you can provide, the easier it is to evaluate the facts and circumstances surrounding the issue.
- Who was involved?
- What did they do?
- When did they do it?
- Where did it happen?
- How did it happen?
- How do you know it happened?
3. Mail your completed form to Tennessee Department of Revenue, Special Investigations, P.O. Box 198466, Nashville, TN 37219, or you can fax it to (615) 532-7835.
