Available to: Recipients of the Bronze Star – Valor
Documentation Required:
- For applicants that have been discharged from the armed forces: DD214 form, along with a copy of orders that awarded the medal or a copy of the certificate or citation granting the medal
- For applicants who have not been discharged from the armed forces: Copy of orders that awarded the medal or copy of certificate or citation granting the medal
- For those who do not have documentation listed above: a written communication from the Department of Veteran Services that certifies the applicant has received the medal or award
Cost: None for first plate. Second plate - $26.50 annual fee (initial fee is $28.15). Subsequent plates - $26.50 annual fee (initial fee is $29.80).
