Money Follows the Person

GRANT OPPORTUNITY:

Money Follows the Person (MFP) is a federal demonstration program to help Medicaid eligible individuals living in qualified institutions transition back into the community. With a commitment to rebalancing and quality enhancements, the Money Follows the Person (MFP) - Community Transition Grants will allow Tennessee’s MFP Demonstration to fully leverage leadership, strategic planning, and partnership opportunities to advance a person-centered system of service delivery for people with I/DD that supports them to increase their independence and fully participate in their communities. DIDD will be providing 3 regionally dispersed agencies that operate Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) and are also Home and Community-Based Services (HCBS) providers the opportunity to consult with subject matter experts to develop transition plans for 3 ICF/IID locations. Grant applications are due May 12, 2023.
