TN Immunization Registry Opt-Out

Tennessee Immunization Registry - TennIIS 

The Tennessee Immunization Information System (TennIIS) is the state's online immunization registry where records of immunizations may be maintained permanently for persons of all ages.  State law authorizes access to immunization information by healthcare professionals and pharmacists, as well as school and childcare personnel responsible for ensuring children meet immunization requirements for participation in school or childcare.  Only legally authorized users who register and are approved by TDH staff may be granted secure access to TennIIS.  

Over a lifetime, many people misplace personal immunization records.  TennIIS records are never deleted and will always be easily accessible to authorized users any time a person needs proof of immunization.  

How to Opt-Out of Having Vaccine Information in TennIIS

An individual or the parent or legal guardian of a child may request that immunization information not be recorded in TennIIS.  Individuals may opt to resume recording immunization information in TennIIS at any time.  

When opting out, an individual's basic identifying information such as name and date of birth, remains in TennIIS.  If any healthcare provider or electronic medical record system attempts to submit vaccine records for that individual, the vaccine data will be unable to update immunization information on that record.  

To begin a TennIIS opt-out request, please contact the TennIIS Help Desk at TennIIS.Help@tn.gov or 800-342-1813.  Once the request has been received, TIP staff will email, fax, or mail an Immunization Registry Change of Status request form that must be completed and signed, and then faxed, emailed, or mailed back to TIP before an opt-out request can be authorized.
