WISEWOMAN

About

WISEWOMAN (Well-Integrated Screening and Evaluation for WOMen Across the Nation) is an extension of the Tennessee Breast and Cervical Screening Program (TBCSP).   

WISEWOMAN seeks to improve delivery of heart disease and stroke prevention to women most in need. Specifically, the program helps participants understand their cardiovascular disease (CVD) risk factors, improve blood pressure awareness and control, and make healthy lifestyle choices.

WISEWOMAN is a federal program funded by the Centers for Disease Control and Prevention (CDC) and administered in Tennessee by the Tennessee Department of Health. 

Qualifying participants can receive FREE services such as: 

  • Blood pressure test
  • Cholesterol test
  • Blood Sugar (glucose or A1c) test
  • Referral to health programs (i.e., health coaching, blood pressure self monitoring, and chronic disease management)
  • Medical referrals and follow-up are provided as appropriate

Who is Eligible for the WISEWOMAN Program? 

  • Female residents of Tennessee
  • Aged 35-64
  • At risk for or living with CVD
  • Currently enrolled in the Tennessee Breast and Cervical Cancer Screening program (TBCSP)

                                                      o  Uninsured or underinsured

                                                       o  Living at or below 250% of the federal poverty level for family size
